- What is Transition and Transfer of Care?
For Youth with Chronic Conditions, Becoming Young Adults
Children’s Hospital Los Angeles offers a Transition Program for teens between the ages of 15 and 21. Our specialists have expertise in teaching young people to actively participate in their own health. They empower teens to take control of their health issues and support them in transitioning their care from Children’s Hospital Los Angeles to an adult medical center, as they reach young adulthood.
For many young people, Children’s Hospital Los Angeles is like a second home. But as our patients grow into young adults, they must prepare for the day when they will leave this home and establish a new medical home with an adult health care provider.

These are the aspects we focus on to prepare for that day, when care is transferred, and to help ensure a smooth transition.

Clinic Visits
At the beginning of the transition process, you will continue to see your regular doctors at Children’s Hospital Los Angeles. You will also see transition specialists who are here to help you and your family make a smooth transition to adult care.
During your visit to our clinic, you’ll receive a physical, psychological and social evaluation. We will also create a comprehensive individualized plan just for you, to address any of the following issues:
- Medication adherence
- Reproductive health needs
- Substance use and abuse
- Overall mental health
- Career/vocation planning
Some of the tools we use in the process include health passports, interactive Web programs, financial counseling, individual therapy and peer support. We also work closely with your family members to prepare them for the crucial role they will play in your transition process.
