Enhanced Care Management

Together with your managed care plan, eligible families can get Enhanced Care Management services. This is a new Medi-Cal benefit through the California Advancing and Innovating Medical (CalAIM) program that will help coordinate care in and out of the hospital. A Community Health Worker (CHW) will help coordinate you/your child’s services with your care team, both at the hospital and in the community.

Enhanced Care Management Eligibility

Patients can get Enhanced Care Management if they:

  • Have Medi-Cal managed care coverage
  • Are within the California Children's Services (CCS) program and have a social need, such as:
    • Behavioral health
    • Financial issues
    • Food
    • Housing
    • Mental health
    • Transportation
  • Need help coordinating care between behavioral and mental health, child welfare, and/or birthing teams.

This is a free program for patients and families that qualify.

How Community Health Workers Help

Community Health Workers (CHWs) are non-licensed professionals who work with families by going to appointments and helping to connect to resources. They know a lot about the community they serve and are trusted. CHWs connect health care, social services, and the community.

CHWs can give support at the hospital, on the phone, by email, or in-person in the community. They are trusted individuals who know a lot about different cultural aspects. CHWs can meet families in the community where they live, can go to appointments, and can help make calls.

Other benefits of Community Health Workers:

  • CHWs can help apply for government benefits. This can be for things like medically tailored meals, housing, help with utilities and the CalFresh Food program.
  • CHWs are easy to get in touch with.
  • CHWs fit their services to your specific needs.
  • CHWs support families and help them understand their care plan and appointments at CHLA.

Community Health Workers do not:

  • Transport patients or families in their own vehicle
  • Act as an interpreter in any setting
  • Provide therapy for patients or families

How Patients and Families Participate

If you enroll in this program, keep in touch with your assigned Community Health Worker. You should connect with them at least once a month.

You and your Community Health Worker will decide how often to meet. This can be in person, by email and over the phone. You can meet at doctor visits, in the hospital, or in the community at coffee shops, community centers, parks, or wherever you prefer.
