Get connected to Children’s Hospital Los Angeles through the myCHLA Provider Portal. This powerful tool enables you to submit and track referrals, even those submitted via fax, and gives you real-time access to your patients’ medical records.
Fax
To refer a patient via fax, please download and complete the Outpatient Referral Form. Fax it to 323-361-8988.
For referrals to Neurology, Pulmonary, and Radiology, please use the forms listed below. Forms can be submitted through the Provider Portal or faxed along with any required documentation. For Fetal-Maternal referrals, visit the link below to learn more about the referral process.

Please note that the following information is required to admit a patient: Agreement from the patient/parent to attempt transfer to CHLA, patient demographics, parent contact information, admitting diagnosis, patient insurance information, and referring physician contact information.
