We are one of the only physician-led association in the region supporting pediatric primary care providers. Becoming a Care Network member helps you make clinical, operational, technical and financial decisions that support your independent practice.

Applying to the Care Network: What to Expect

If you are interested in becoming a Care Network member, here’s how the application process works:

  1. Contact us by calling 323-203-7041 or emailing PrimaryCareNetwork@chla.usc.edu.
  2. Complete a practice assessment questionnaire.
  3. Meet with the Care Network team to learn more about membership.
  4. Sign contracts.
  5. Participate in an orientation.
